Role Description The Care Coordinator is a full-time, remote role responsible for overseeing the delivery of home care services to clients and their families. Day-to-day tasks include assessing client needs, developing care plans, coordinating services, and monitoring outcomes to ensure high-quality, person‑centred care. The Care Coordinator will liaise with clients, families, and multidisciplinary teams, manage case documentation, schedule and adjust services, and respond promptly to inquiries or concerns. The role also involves maintaining compliance with organisational policies and relevant regulations, supporting continuous improvement initiatives, and promoting a positive client experience through clear communication and proactive problem-solving.
Qualifications
  • Demonstrated skills in Care Coordination and Case Management to plan, implement, and review client care.
  • Experience in Home Care service delivery, with understanding of community-based supports and aged care needs.
  • Strong Communication and Customer Service skills to engage effectively with clients, families, and care teams.
  • Ability to work remotely in a full-time capacity, managing time, priorities, and digital records efficiently.
  • Relevant qualification in social work, nursing, allied health, or community services (or equivalent experience).
  • Knowledge of aged care standards, client-centered care principles, and safeguarding practices.
  • Proficiency with care management systems and basic office software for documentation and reporting.
  • Capacity to handle sensitive information confidentially and exercise sound judgment in complex situations.
