About the RoleAs a Support Coordinator, you’ll assist participants to build capacity and achieve their NDIS goals by:
- Taking a holistic view, coordinating services, and maximising plan outcomes.
- Building and maintaining proactive relationships with providers, hospitals, and community-based services.
- Ensuring participants receive value-for-money supports while encouraging self-direction.
- Advocating with mainstream services (housing, health, justice, AOD) to meet obligations.
- Monitoring participant progress, plan spend, and maintaining clear, accurate records.
- Staying up to date with NDIS changes and engaging in continuous learning.
This role is Independent Contractor-based, meaning you manage your own tax, ABN, and insurances.
About You
We’re looking for Support Coordinators who are:
- Proactive, resilient, adaptable, and able to work autonomously.
- Experienced in working with complex mental health, hospital discharge, NGO housing, or AOD-related cases (highly regarded).
- Well-connected within their professional networks and able to bring participants through those relationships.
- Skilled communicators with strong relationship management ability.
- Able to manage compliance requirements while staying participant-focused.
Requirements:
- ABN & relevant insurances (Public Liability + Professional Indemnity).
- Valid NDIS Worker Screening Check.
- Relevant Working With Children Check (state-based).
- Driver’s licence and reliable, insured vehicle.
- Copies of relevant qualifications/certifications.
